Cornell Scott
1935 - 2008

Cornell Scott, Chief Executive Officer of the Hill Health Corporation, died Monday, August 25, 2008 after a protracted illness. He was 73 years old.

Mr. Scott was born on April 11, 1935, a native of Holly Grove, AR. He was the son of the late Tommy and Leola Thomas Scott.

He professed Christ at an early age and became a member of the New Rising Star Baptist Church in Holly Grove, AR.

He obtained his Bachelor’s Degree from the University of Arkansas and did Graduate Study at the Universities of Oklahoma and Wisconsin. He earned a Master’s Degree in Public Health from the Yale School of Medicine in 1968.

Mr. Scott came to the Hill Health Center (HHC) when it opened in 1968 as Director of Training and Education and was appointed Executive Director in 1972.

His 40-year tenure at the Hill Health Center was marked by steady development of services, sites and programs aimed at the low-income population of the New Haven area. Mr. Scott earned a national reputation through his work with the National Association of Community Health Centers (NACHC). He was president of the Association in the 80s when the health center program faced extinction in the U.S. Congress. His work is attributed with helping to preserve national funding, leading to the growth of community health centers throughout the nation.

He was a founder and first president of the Community Health Center Association of Connecticut, as well as founder and Chairman of the Board of Directors of Community Health Network of Connecticut, a non-profit statewide managed care organization that serves the Medicaid population.

He gave freely of his time to foster the spirit of public health and served on numerous boards and professional organizations, including Connecticut Hospice, the Connecticut Hospital Association, Gaylord Hospital, The Community Foundation for Greater New Haven, Yale-New Haven Hospital, the United Way, the New Haven Housing Authority, New Alliance Bank, the New Alliance Foundation, the Hill Development Corporation, and the Universal Health Foundation.

In 2002, he received the National Association of Community Health Center Association Lifetime Achievement Award for his “lifelong dedication and exceptional service to the American people.” With his long litany of lifetime accomplishments, “Scotty, ” as he preferred to be called, is perhaps best remembered for his graciousness, his ability to bring people together to address the needs of community health, and his passionate advocacy for the health care needs of the poor.

He is survived by his brother, Hubert (Jo Ann) Scott of Los Angeles, CA; nieces Roslyn Scott of Arlington, TX, Sharon (Rickey) Sain of Little Rock, AR, Mayola Scott Wilson of Kansas City, MO, and Sandra Williams of St. Louis, MO; nephews, Hubert Cornell Scott of Los Angeles, CA, Stanley Scott of Holly Grove, AR, Gene Edward Scott II of Phoenix, AZ, Kevin Wayne Scott of Memphis, TN and Kenneth Ray Smith of Little Rock, AR; and a host of cousins, great nieces and nephews.

His sister Mary Allene Smith, brothers Herman and Gene Edward Scott, and nephew Tommy Edward Scott preceded him in death.

He attended the Arkansas – Pine Bluff Homecoming events until he became ill and kept in touch with his relatives; he loved to attend his family reunions.

A celebration of Mr. Scott’s life and legacy will be held Wednesday, September 3, 2008, at 10:00 AM at Battell Chapel, 400 College Street, New Haven. Interment will be in Grove Street Cemetery. Friends may call at Battell Chapel from 8:00 – 10:00 AM prior to the service and on Tuesday, September 2, 2008, from 4:00 – 8:00 PM at Immanuel Missionary Baptist Church, 1324 Chapel Street, New Haven.

In lieu of flowers, memorial contributions may be made to either the Cornell Scott Scholarship Fund at The Community Foundation for Greater New Haven, 70 Audubon Street, New Haven, CT 06510, which supports minority students attending Yale’s School of Epidemiology and Public Health or to the Hill Health Center Raymond P. Wong Pharmacy Fund, c/o Robert Kilpatrick, 400 Columbus Avenue, New Haven, CT 06519.

Barbara D. Simmons
1943 - 2009

Barbara D. Simmons, 66, of West Haven, departed this life March 7, 2009.

Born in Humphrey, AR, January 6, 1943, she was the daughter of the late James Henry Donaldson and Bernice Price Donaldson.

Mrs. Simmons was most recently employed at Yale University Divinity School as an Administrator and was previously employed by the State of Connecticut Department of Social Services as a Fiscal Services Officer.

She is survived by a daughter, Danika Simmons; a granddaughter, Jazmine Gordino; sisters, Patricia D. Turner (Richard), Ellen D. Doram; and Sara T. Greaux; a cousin, Yvonne B. Gorham; and a host of nieces, nephews, other relatives and friends.

She was preceded in death by brothers, William H. Donaldson, and John H. Donaldson.

She is also survived by her husband Terrence Simmons.

A celebration of Mrs. Simmons' life and legacy will be held Friday, March 13, 2009, at 11:00 a.m. at Immanuel Missionary Baptist Church, 1324 Chapel St., New Haven. Friends may call at the church from 10:00-11:00 a.m. Entombment will be in the Beaverdale Mausoleum.

In lieu of flowers, memorial contributions may be made to Connecticut Hospice, 100 Double Beach Rd., Branford, CT 06405.

Edythe M. Crosier

Edythe M. Crosier of New Haven, died November 20, 2006 at her home. She was the wife of Douglas Crosier.

Born in Orange, CT, she was the daughter of the late George F. and Edith Boothe.

In addition to her husband she is survived by a host of nieces and nephews. She was predeceased by several brothers and sisters.

A memorial service celebrating Mrs. Crosier's life and legacy will be held Saturday, December 2, 2006, at 11 AM at Kingdom Hall of Jehovah's Witnesses, 885 Wintergreen Ave, Hamden.

Dolores E. Bush
1946 - 2010

Dolores E. Bush, 64, of New Haven, departed this life October 23, 2010.

Born in Pitt County, NC, October 21, 1946, she was the daughter of Gaytha Mae Bush and the late Clifton T. Bush, Sr.

Ms. Bush was employed at the Agency on Aging as a Case Manager and was a member of Varick AME Zion Church.

In addition to her mother, she is survived by a son, Erik Bush, brothers, Clifton T. Bush, Jr., Curtis R. Bush (Debra), Edward W. Bush (Trina) and Gary T. Bush (Sherry); sisters, Delzora and Willie B. Bush and Angela Pouncey; seven grandchildren; and a host of nieces, nephews, other relatives and friends.

She was preceded in death by a sister, Bertha Bush.

A celebration of Ms. Bush's life and legacy will be held Friday, October 29, 2010, at 11:00 AM at St. Paul UAME Church, 1267 Chapel St., New Haven. Viewing and visitation will be held at the church from 10:00-11:00 AM. Burial will be in Beaverdale Memorial Park.

Henry P. Jones
1953 - 2007

Henry P. Jones, 53, of Hartford, departed this life July 31, 2007. He was the widower of Lela Jones.

Born September 12, 1953 in San Antonio, TX, he was the son of Francene Jones and the late Scott Jones.

In addition to his mother, he is survived by siblings, Scott, Leroy, Melvin, Steve, Carol and Audrey Jones and Edna Adkins.

He was predeceased by a son, Lamont Jones.

A celebration of Mr. Jones' life will be held Wednesday, August 8, 2007, at 11:00 AM at Curvin K. Council Funeral Home, 128 Dwight St., New Haven. Friends may call from 10:00-11:00 AM. Burial in Evergreen Cemetery.

Allen Tart
1976 - 2007

Allen Tart, 31, of Hamden, departed this life July 5, 2007.

Born in New Haven, CT, April 13, 1976, he was the son of Joanne Tart and Allen Jones.

In addition to his parents, he is survived by a brother, Erwin Tart; sisters, Gwendaline Tart and Matilda Anne Barrett; his maternal grandmother, Julia Holland; aunts, uncles, other relatives and friends.

He was predeceased by his grandfather, Ben Frank Tart and his stepfather, Erwin Glenn Sheppard.

A celebration of Mr. Tart's life will be held Thursday, July 12, 2007, at 11:00 am at Curvin K. Council Funeral Home, 128 Dwight St., New Haven. Friends may call from 10:00-11:00 am. Burial in Hamden Plains Cemetery.

The Honorable Constance Baker Motley

The Honorable Constance Baker Motley, died on September 28, 2005, in New York City.
Deeply mourned by her loving husband, Joel Motley, Jr.; her son, Joel Motley III (Isolde); her grandchildren, Hannah, Ian and Senay; her sisters, Edna Carnegie, Eunice Royster and Marion Green; her brother, Edward Baker; and her many cousins, nephews, nieces and friends.

Amistito Rivera-Maldonado
1943 - 2006

Amistito Rivera-Maldonado, 63, of West Haven, died August 17, 2006.

Amistito was the son of Clementina Maldonado and the late Guadalupe Rivera.

In addition to his mother, he is survived by his companion, Rosemary Rivera; children, Elmer, Monica, Llesenia and Veronica Rivera; 10 grandchildren and other relatives and friends.

A funeral service celebrating Amistito's life and legacy will be held Monday, August 21, 2006, at 9 AM at Curvin K. Council Funeral Home, 128 Dwight St., New Haven. Burial in St. Lawrence Cemetery. Friends may call Sunday from 6-8 PM.

Ralph Earl Hylton
1941- 2005

Ralph Earl Hylton entered eternal rest on September 24, 2005.

Mr. Hylton was born in New Haven, CT, December 15, 1941, son of the late Rudolph A. and Hazel W. Hylton.

He was a 1960 graduate of Hillhouse High School, served in the U.S. Army and was most recently employed by FedEx-Kinko.

He leaves behind his close friend and confidant, Dawna Daniel of New Haven with whom he resided; a daughter, Michelle and son-in-law, Seth Soronnadi of New Haven; two brothers, Lionel A. Hylton of Severn, MD and twin, Rudolph A. and his wife Melvenia Hylton of New Haven; nieces, nephews, cousins and a host of good friends.

Funeral service will be held Friday, October 7, 2005, at 11am at Curvin K. Council Funeral Home, 128 Dwight St., New Haven. Friends may call from 10am-11am. Burial in Beaverdale Memorial Park.

Gwendolyn Mazyck
1971 - 2008

Gwendolyn Mazyck, 37, of North Haven, departed this life July 23, 2008.

Born in New Haven, CT, June 9, 1971, she was the daughter of James Mazyck and Mattie Mazyck.

In addition to her parents, she is survived by a son, Louis Mazyck; daughters, Tyneisha, Tyshawna and Jennaya Mazyck; a brother, Eldridge Mazyck; and a host of other relatives and friends.

She was preceded in death by a brother, Andre McKnight.

A memorial service to celebrate Gwendolyn's life and legacy will be held Monday, July 28, 2008 at 6:00 PM at Curvin K. Council Funeral Home, 128 Dwight St., New Haven.

Carlton B. Woods
1966 - 2008

Carlton B. Woods, 41, of Meriden, departed this life July 2, 2008.

Born in New Haven, CT, August 23, 1966, he was the son of Harry L. and Dora Murphy Woods.

Mr. Woods was employed at R&L Trucking Co. as a Truck Driver.

In addition to his parents, he leaves to cherish his memories his children, Jerri, Miles and Chardia Woods; brothers, Calvin Woods and Alonzo Woods (Patricia); sisters, Sylvia Woods and Chickqueetta B. Jenkins; his companion, Latrelle Mitchell; and a host of aunts, uncles, other relatives and friends.

A celebration of Mr. Woods' life and legacy will be held Tuesday, July 8, 2008, at 11:00 AM at Curvin K. Council Funeral Home, 128 Dwight St., New Haven. Friends may call from 10:00-11:00 AM. Burial will be in Beaverdale Memorial Park.
